Comments:
An unannounced Monitoring Inspection was conducted today from 3:40pm to 4:40pm. There were 17 school-agers directly supervised by 2 staff. The physical plant, staff records, 6 children?s records, 0 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, individual activities and socializing. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that written records of children's serious to minor injuries contained the required information.
Evidence: Reviewed 4 written injury records and found that 2 did not have documentation of time the parents were notified.

Will ensure the time is added going forward.
