Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on December 7, 2020 and concluded on December 11, 2020. The program director was contacted by telephone to initiate the virtual, remote inspection. There were 23 children present with two staff. The inspector reviewed a list of items required to complete the inspection. The inspector reviewed three children?s and four staff records during the virtual, remote inspection to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview the center failed to obtain a physical for each child as required. Evidence: 1 - The record for child 1 does not contain documentation of physical. Child 1 has an undetermined date of enrollment. 2-The requested information was not included with other documents received regarding child 1 which were submitted by the administrator.
Plan of Correction:
Documentation of the physical and enrollment date for child 1 will be obtained by the facility

Description:
Based on record review and interview the center failed to obtain a central registry finding within 30 days of employment as required. Evidence: 1 - The record for staff 1 does not contain documentation of a central registry finding. Staff 1 has documented date of hire as October 3, 2019. 2 - When requested the documents submitted for staff 1, by the administrator did not include documentation of central registry finding for staff 1.

Description:
Based on record review the center failed to obtain required fingerprints prior to employment. Evidence: The record for staff 2 contains fingerprint results dated June 5, 2019. However, the documented date of hire for staff 2 is recorded as April 4, 2019. Therefore, the fingerprint results were obtained after the date of employment.
Plan of Correction:
In the future the facility will ensure that the required fingerprint background checks are obtained prior to employment.
