A subsidy inspection was initiated and concluded on November 13, 2023. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.
Based on record review and interview the vendor failed to ensure that attendance records are maintained as required. Evidence: 1 - The attendance record available for review were not hard copy and did not document arrival and departure as it occurs. 2 - Staff #1 confirmed that there was not a attendance documentation in the required form.
