An unannounced complaint investigation was initiated on 06/03/2024 from 10:25am to 10:30am as the result of a complaint received in our office regarding the operational responsibilities of the Licensee. The Licensee was not onsite to interview at the time of the investigation, though they were able to be reached by phone the same day. Based on a preponderance of evidence, the complaint was determined to be valid.

Based on interview, the center did not ensure that they maintain public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate. Evidence: Upon interview of the Licensee, they stated that the liability insurance policy had expired on 5/8/24 and had not been renewed.
Plan of Correction:
Insurance was canceled on 05/08/24 because the new owners had already obtained insurance under their policy. After the inspectors visit, we began the process to get the current licensee reinsured under the said criteria. In addition, we also added the current license to the owners policy for extra pre-caution. Began the liability insurance process.
