Time frames for staff TB screening was discussed .
Comments:
An unannounced subsidy health and safety inspection was conducted on 11/04/2019 from 4:45 pm - 5:51 pm. Four staff records were reviewed at the Blocker YMCA on 11/05/2019. At the time of the facility review there were 23 school age children in care with two staff. Children were observed during late afternoon program time which included table games and coloring pictures. Records were reviewed for eight children in care. Medication and emergency suppliers were also reviewed.
One violation was observed in the area of staff records and is listed on the violation notice. This violation was discussed with administrative staff at the Blocker Family YMCA during the exit interview for the staff record review on 11/05/2019.
Based on record review, the vendor failed to ensure that the documentation of the tuberculosis screening shall have been completed within the last 30 calendar days prior to employment.
Evidence: 1. The TB screening on file for staff 1 was completed on 03/28/2019. Staff 1 has a hire date of 10/12/2019. 2. The TB screening on file for staff 2 was completed on 10/05/2019. Staff 2 has a hire date of 09/28/2019. 3. The TB screening on file for staff 3 was completed on 10/19/2019. Staff 3 has a hire date of 09/03/2019.
Plan of Correction:
Staff #1 is scheduled to obtain a TB screening on 11/11/2019. Results will then be obtained and filed in staff file by 11/15/2019. Moving forward, if a staff is rehired or transferred from another department or Family Center, their staff records must be reviewed prior to their rehire date and first date on the floor. If TB test results are not within the time frame, staff will be sent to receive a TB test screening. These results must be obtained and have a negative result prior to working on the floor. The program administrator and director will be responsible for these actions.
