Comments:
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 11/13/2020 and concluded on 11/20/2020. The director was contacted by telephone to initiate the inspection. There were 5 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 staff records and 2 children's records submitted by the facility to ensure documentation was complete.

The information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the director COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-280-G
Description: Based on observation, the center failed to ensure if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents.

Evidence:
1. One bottle of disinfectant was not labeled to indicate contents.
a. Staff identified the contents as disinfectant 264.

Plan of Correction: The bottle was labeled during the inspection.

Standard #: 22VAC40-185-510-A
Description: Based on record review and interview, the center failed to ensure that medication shall be given only with written authorization from the parent.

Evidence:
1. One prescription medication, for child 1, lacked written authorization from the parent for administration.
a. This medication has been administered 21 times between 10/13/2020 and 11/13/2020 according to written medication logs.
2. Administrative staff confirmed that the parent had not signed the medication authorization form.
a. The child's physician did sign the medication authorization form on 07/01/2020.

Plan of Correction: The parent will be asked to sign the medication authorization form.
