09/16/2019 Arrived 11:00am Departed 12:15pm An unannounced monitoring inspection was conducted today. Thirty-two children were present with four staff. The children were playing outdoors, reading a story in group time and eating lunch. Five children's files and two staff files were reviewed. Based on a tour of the outdoor play area, the facility failed to ensure that all equipment is safe. Evidence: 1. On the stationary car, the wooden seat is worn and presents splinters.
