Technical Assistance:
Technical assistance was provided in the following areas; retaining children?s records, lockdown drill documentation, labeling of water cups and medication.

Comments:
An unannounced renewal inspection was conducted on 8/21/2023 from10:00am to 12:15pm. At the time of the inspection there were 75 children in care and 10 staff members present. The sample size consisted of 6 children?s records and 5 staff records. Children were observed during outdoor play, snack time and engaged in indoor activities. Medication and evacuation drills were reviewed during the inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of 5 staff records it was determined the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening prior to employment beginning.
Evidence:
1. The record for Staff #1 did not contain documentation of a tuberculosis (TB) screening.
2. The Director confirmed the record for Staff #1 did not contain documentation of a TB screening.

Plan of Correction: The staff member obtained her TB screening 8.21.2023 and documentation confirming screening was submitted to inspector on 8/25/2023.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interviews, it was determined the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
Evidence:
1. In the bathroom of the Ladybug classroom there was insulation protruding from a hole in the wall next to the toilet. The hole was approximately 10? wide and the insulation was accessible to children.
2. Staff #6 confirmed there was a hole in the wall accessible to children.

Plan of Correction: The bathroom hole was covered and insulation was no longer accessible on the date of the inspection.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation and interviews, it was determined the center did not ensure that electrical outlets have protective covers.
Evidence:
1. In the Ducklings classroom there was an electrical outlet without a protective cover in an open floor cabinet. This cabinet was accessible to children.
2. Staff #6 confirmed there was no protective cover on the outlet.

Plan of Correction: Safety plugs were placed in the electrical outlet on the day of the inspection.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation and interviews, it was determined the center did not ensure that for children, from three years old to the age of eligibility to attend school, one staff member for every 10 children is maintained when children are in care.
Evidence:
1. The four year class was on the playground with Staff #3. There were eleven children and one staff member supervising. The center did not maintain the 1:10 ratio required for four year old children.
2. Staff #3 confirmed the class was not in the 1:10 ratio.

Plan of Correction: The ratio was immediately corrected during the inspectors visit.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interviews, it was determined the center did not ensure that when food is brought from home it is clearly dated and label in a way that identifies the owner.
Evidence:
1. In the infant room refrigerator there were four bottles containing formula or breast milk that did not have dates on them.
2. The Director confirmed the bottles did not have dates.

Plan of Correction: Infant bottles are now to be labeled daily with the dated stickers printed by the school.
