Comments:
An unannounced monitoring inspection was completed on 10/30/2019. There were 125 children, ages 3 years-5 years, 17 staff providing direct care and supervision and numerous support staff present during the inspection; 62 children were on a field trip during the inspection. The inspector reviewed 10 children's records, 10 staff records and 5 medications during the inspection. Children were observed in the following activities: during learning time, participating in an unannounced fire drill, in free choice activities and during lunch. The inspector reviewed the new playground equipment and emergency supplies during the inspection. The inspector discussed hand washing and playground equipment with the staff. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and discussion with the staff, the facility failed to obtain documentation of a negative tuberculosis screening no later than 21 days after employment.

Evidence:
1. The record for Staff #7 has a documented hire date of 06/10/2019; the date on the tuberculosis screening is 07/11/2019.
2. The record for Staff #7 has a documented hire date of 08/05/2019; the date on the tuberculosis screening is 09/04/2019.

Plan of Correction: Will attempt to get all future TB in the required time.

Standard #: 22VAC40-185-280-B
Description: Based on observations and discussion with the staff, the facility failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: There was a container of DampRid unlocked on top of a storage cabinet in Classroom #6.

Plan of Correction: Was removed during the inspection.
