Comments:
A monitoring inspection was conducted on February 27, 2023 from 1:40 P.M.-2:15 P.M. at the administrative office and from 3:20 P.M.-5:00 P.M. on site of the program. There were 22 children present, ranging in ages from five to eleven years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on interviews and review of children records, the center failed to ensure that all required information was in the file.

Evidence:
1. Child #4's file did not contain the date of the child's first date of attendance.
2. Staff #1 and #2 verified that the record did not contain the child's first date of attendance.

Description: Based on observation of posted information, the center failed to ensure that the Superintendents Special Order issued by the department was posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.

Evidence:
The Special Order provided by the Department of Education, signed by the director of the Office of Child Care Health and Safety on 11/05/2022, was not posted in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations.
