Technical Assistance:
Technical assistance was provided regarding the 16 hour annual training requirement for staff. In addition, please ensure the updated Sworn Disclosure Statement form is being used.

Comments:
An unannounced monitoring inspection was conducted today from 10:35am-12:45pm. There were 33 children directly supervised by 5 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, 3 staff records, 7 children's records, emergency drills, injury reports, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing with puzzles, blocks, and cars, coloring, learning about the letter "i" and working on an igloo craft project. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on observation, children's food brought from home was not dated.

Evidence:
1. Observation was completed in both the 3 year old and 5 year old classrooms. Children's lunch boxes stored in cubbies were observed to be labeled with the children's names but were not labeled with the date.

Plan of Correction: The date will be put on each lunch box going forward.
