Comments:
An unannounced monitoring inspection was conducted on 7/24/19 from 10:10am - 1:10pm. At the time of entrance there were 86 school age children in care with 12 staff. Children were observed participating in various activities in the gym and the classrooms, swimming in the pool, eating lunch, and playing outside. Records were reviewed for nine children and nine staff during the inspection. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of nine children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not indicate whether or not the child had previously attended child care. 2. The record for child #4 did not indicate whether or not the child had previously attended child care. 3. The record for child #5 did not indicate whether or not the child had previously attended child care. 4. Staff #10 (Program Director) confirmed that the records for child #1, child #4 and child #5 did not contain all of the required information.

Plan of Correction: The faiclity responded: The parents of the children will be asked to provide the missing information. All new children will have their records reviewed prior to enrollment to ensure they are complete.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of nine staff records, it was determined that the facility did not ensure that an employee is allowed to begin employment without a completed national criminal history record check (finger printing). Evidence: 1. The record for staff #2 (date of hire 6/17/18) did not contain documentation of a completed national criminal history record check (finger printing). 2. Staff #10 (Program Director) reviewed the record for the staff #2 and confirmed that she had not received the results of the finger print background check (finger printing).

Plan of Correction: The facility responded: Staff #2 had previously completed a finger print background check, but we are unable to find it. We will have staff #2 complete a new background check. all new staff will have a finger print background check in their record before they start working.
