Comments:
A compliance inspection to monitor the requirements for Religiously Exempt Child Day Centers was conducted on 09/12/2019 from 11:00 a.m to 1:00 pm. At the time of arrival the following staff to child ratios were observed:
10 children, three years old, with 1 staff
10 children, three years old, with 2 staff
7 children, two and a half years old, with 2 staff
9 pre-k children, four years old, with 1 staff
9 pre-k children, four years old, with 1 staff
10 kindergarten children, five years old, with 1 staff
Classroom observations were made and children were observed participating in a variety of activities to include story time and discussion, interacting in learning centers and palyin outdoors. Fifteen staff records were reviewed, handbooks and posted information were reviewed and the center contact staff was interviewed regarding center policies and procedures. The facility, to include all indoor areas used by children in care, as well as the outdoor play space were observed.
Staff report that the program does not transport children in a vehicle while in care.

Description: Based on observation and interview, the religious exempt facility has not ensured that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment.
Evidence:
The outdoor play space contained a modular piece of equipment which included 2 slides and stairs, a spring based teeter totter and 3 spring based rocking horses.
These pieces of play equipment were located over grass and did not have sand or other cushioning equipment under it.

Plan of Correction: The director stated that she has already had plans approved to have a new playground installed which includes poured in place resilient surfacing. The church is currently having construction done and the playground was delayed as a result. The director anticipates having resilient surfacing in place by spring. In the mean time staff will vigilantly supervise children to ensure the safest play and children may also utilize the fellowship hall for gross motor play.
