Comments:
An in person inspection was conducted today. All rooms were in ratio during the inspection. The playground was not inspected due to inclement weather. 3 staff records reviewed were complete. 2 additional staff records were missing an annual Health Statement. 1 medication was reviewed and was in compliance.

Based on a review of records, two staff records do not contain all required documentation. Evidence: 2 staff records do not contain documentation of a current Staff Health Report. (Staff A, B)
Plan of Correction:
The staff have had the forms completed; however, they need to pick them up from the doctor's office.
