Comments:
A monitoring inspection was initiated on 6/23/2023 at approximately 1:10pm and concluded on 6/23/2023 at approximately 1:25pm. There were no children in care. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review, and confirmed by center director, the center did not ensure that the criminal history background check was completed every 5 years.
Evidence: The criminal history background check for Staff #4 expired 5/1/2023.

Center will ensure the criminal history background check is completed for Staff #4.
