Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including an in-person component, telephone contacts, documents review, interviews.

A renewal inspection was initiated on 3/24/22 and concluded on 3/25/22. The director was contacted by telephone. The in-person component was conducted on 3/24/22. There were 17 children present, ranging in ages from three to five years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on a tour of the playground, the facility failed to ensure that all equipment is safe.
Evidence:
1. There is a 1/2 inch gap on the slide where it attaches to the green hood, presenting an entanglement point.
2. There is a loose bolt on the under side of the climber near the beige stairs.

Plan of Correction: Work order placed with maintenance department to address the gap on slide and the loose bolt. [sic]

Standard #: 8VAC20-780-330-B
Description: Based on a tour of the playground the facility failed to ensure that resilient surface material complies with minimum safety standards.
Evidence:
1. There are three inches, of the required six inches, of resilient surface material at the base of the straight green slide.
2. There is less than one inch of resilient surface material under the beige steps on the climber.

Plan of Correction: Working to obtain quotes from contractors to address drainage issues as well as slope on playground. Resilient surfacing depth will be addressed as well. Children are playing at an alternate playground until the work is completed. [sic]
