Technical Assistance:
Discussed that one child in the classroom was trying to hide from the teachers, and was out of sight of the teachers, but was observed by the inspector and the assistant director. Discussed with the assistant director that the staff needed to ensure that all children must be under direct supervision of the classroom teachers at all times.

Comments:
An unannounced monitoring inspection was conducted on 12/11/2023 from 11:15am to 12:30pm. There were 13 (preschoolers) directly supervised by 2 staff. The physical plant, 4 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in circle time. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Description: Based on review, the facility did not ensure that they obtained documentation of a physical examination for each child in care 30 days after the first day of attendance.
Evidence: reviewed 5 children's records and found that Child 1 and 3 did not have documentation of a physical examination.

Description: Based on review, the facility did not ensure that children's records maintained had the minimum required information.
Evidence: reviewed 5 children's records and found the following:
1) none of the emergency contacts listed had an address.
2) child 4 and 5 did not have documentation of a second emergency contact.
