Technical Assistance:
We reviewed the requirements for updating immunization records for children. Also, we talked about use of the sanitizing solution during diaper changes.
Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 9:46 AM to 5:18 PM. Today, there were one-hundred and twenty children, ages infancy through school age, with eighteen staff. I viewed program activities, staffing, supervision, interactions with children, equipment, infant care and records, diapering procedures, posted information, emergency supplies and drill records, injury records, menu, nutrition, medication, ten records for children, fifteen staff records, staff qualifications, staff training, agency inspections, daily routines, indoor and outdoor areas. The children were provided a variety of interest areas in the classrooms and age appropriate toys and materials. There have been regular group story times, discussions and planned activities. The children have enjoyed outside experiences. The staff have participated in training opportunities at the center and online.

Based on a review of ten records for children, and an interview with administrative staff, the center failed to obtain documentation of additional immunizations at least once between the fourth and sixth birthday for one child. Evidence: Child 9 turned six years old in December 2019 and the last immunization report in the file was from September 1, 2015.
Plan of Correction:
Administrative staff have requested the updated immunization record. The last request to the parent was the morning of January 6, 2019. The immunization record will be obtained for the record of child 9. A system for reminding parents and tracking the need for updated immunization records will be used.
