An unannounced monitoring inspection was initiated on 04/09/2024 at 1:00 p.m. and was concluded on 04/18/2024 at 4:30 p.m. regarding a self-report that was received on 04/09/2024. The inspector reviewed compliance in the areas of physical plant, staffing and supervision, special care and emergencies.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
