Comments:
An unannounced complaint inspection was initiated on 04/02/2024 at 1:05 p.m. and was concluded on 04/24/2024 at 11:00 a.m. in conjunction with local child protective services. The complaint was received on 03/26/2024 regarding administration, staff qualification and training, staffing and supervision, programs, special care provisions and emergencies and special services. Seven staff members were interviewed and eight classrooms where observed.
Information gathered during the inspection supports at least one of the allegations and the complaint was determined to be valid. Violations were documented on the violation notice issued to the program.

Complaint Related Based on observation and interview, the center failed to maintain the required staff:child ratio of 1:10 for children three years up to school age eligible. Evidence: 1. In the school age classroom, there were 12 children with Staff #8. Staff #8 confirmed that the age of the youngest child was three years old.
Plan of Correction:
There were enough staff on hand to maintain ratio. The other staff member reported to the classroom within minutes.

Description:
Complaint Related Based on review of documentation and interview, the center failed to ensure that when a child?s diaper becomes wet or soiled, the child is changed and clean immediately. Evidence: 1. Review of diapering logs showed the following: 1) Between the dates of 03/04/2024 and 03/22/2024, there were seven occasions that Child #1?s diaper changes were three to six hours apart. 2) Between the dates of 03/13/2024 and 03/27/2024, there were four occasions that Child #3?s diaper changes were four to seven hours apart. 3) Between the dates of 03/04/2024 and 03/17/2024, there were eight occasions that Child #4?s diaper changes were three to nine hours apart. 4) Between the dates of 03/04/2024 and 03/27/2024, there were five occasions that Child #5?s diaper changes were three and a half to five hours apart. 2. When asked, Six of the seven staff members stated that the center?s policy is that diaper changes are to be at least every two hours or as needed. 3. Interviews revealed diapers aren?t changed in a timely manner, feces are dry/caked on/hard to wipe, diaper rashes have been noticed, and multiple parents have complained about diapers being full at pick up.
Plan of Correction:
If the diapers of the two year olds were dry upon checking and potty sitting, the staff were not always documenting. There were also times when the checks went longer because they were checked before lunch and not again until after nap (lunch and nap are back to back). A retraining of diaper changing/potty training was conducted with the staff on 4-24-2024.
