Comments:
The licensing inspector conducted an unannounced complaint investigation in response to a complaint that was received by the Office of Child Care Health and Safety on March 1, 2023, regarding allegations in the area of ratio. The investigation was initiated on March 7, 2023 and concluded on March 7, 2023. On March 7, 2023, the inspector was on site at the center from approximately 8:35am to 9:30am. Interview?s with the center representatives and three staff members were completed, observations were made in the center, and digital documentation was reviewed.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Based on interviews, the center did not ensure to employ supervisory personnel according to the following ratio of staff to children: One staff member to eight children from ages 24 months to 36 months. Additionally, the center did not ensure the age of the youngest child in the group be used to determine the staff-to-children ratio that applies to that group. Evidence: During the March 7, 2023, inspection, the following was determined: 1. Upon arrival at the center at approximately 8:35am, the licensing inspector observed 22 children, ranging from age two years to five years, being supervised by 2 employees. Later during the inspection at approximately 8:50am, the licensing inspector observed 26 children, ranging from age two years to five years, being supervised by 2 employees. The licensing inspector also observed 9 two-year-old children in another classroom being supervised by 1 employee at approximately 8:40am.
2. Two staff members reported the following: a. Approximately 3 out of 5 days per week prior to 7:30am, staff #1 is out of ratio and supervising more than 8 children. b. Staff #1 supervises all children ranging in ages two years to five years who are dropped off in classroom C-134 daily until 7:30am when staff #2 arrives.
