An unannounced monitoring inspection was conducted on September 20, 2019 from approximately 9:00am-1:00pm. There were 16 children present with two staff members. The children were observed arriving to and departing from the program, during circle time, outdoor play and free play, participating in a fire drill and coloring worksheets. Two classrooms, two restrooms, the gymnasium, vehicles used for field trip transportation and the outdoor playground area were reviewed. Two staff records were reviewed.
Based on record review, in two of two staff records reviewed, the center did not ensure that staff have a completed sworn statement or affirmation before employment and a Search of the Central Registry within 30 days of employment.
Evidence: The Director confirmed that there were was not a Central Registry finding and no documentation of a completed sworn statement or affirmation in the records for Staff #1 (date of hire-8/2/16) and Staff #2 (date of hire-8/19/19).
Plan of Correction:
Both staff members completed sworn statements while the inspector was present. Both staff members will complete and mail the Central Registry forms and provide a copy to the licensing inspector.

Based on two of two staff records reviewed and staff interviews, the center did not ensure that staff submit to fingerprinting and obtain results by September 30, 2018 or prior to employment.
Evidence: The Director confirmed that the fingerprint results letter for Staff #1 (date of hire 8/2/16) was dated 8/28/19 and the fingerprint results letter for Staff #2 (date of hire 8/19/19) was dated 8/30/19.
Plan of Correction:
If we have to hire new staff we will get the fingerprint screening prior to employment.
