This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on October 14,2020 and concluded on October 15, 2020. The provider was contacted by telephone to initiate the inspection. There were 39 children present and six staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed three children?s records, two staff records, and two board members records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
Based on a record review, the center did not obtain a sworn statement prior to hire.
Evidence: The record for staff #2 (start date 7/01/20), had documentation of a sworn statement dated 7/31/20.
