Comments:
An unannounced monitoring inspection was conducted on 05/11/2023. There were 29 children, ages 3 years-5 years, and 4 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, and lunch. A total of 5 children?s records and 4 staff records, were reviewed. The inspector discussed the following with staff: medications requirement, new location and application process. The inspector arrived at 10:30 AM and departed at 12:01 PM.

Description: REPEAT VIOLATION


Based on review of five children?s records, the center failed to ensure that all records were complete.

Evidence:

The enrollment form did not have a place for chronic physical problems to be documented.

Plan of Correction: The administration will add chronic problems to the new enrollment form for fall.

Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that all chemicals were locked.

Evidence:
In the bathroom, the chemicals were stored in a basket on a shelf and were unlocked.

Plan of Correction: The staff will remove the unlocked chemicals and store them in a locked cabinet.

Description: REPEAT VIOLATION

Based on observation, the center failed to
ensure that all medications were locked.

Evidence:

1. There was an inhaler for child # 5 that was on a shelf unlocked in the director?s office.

Plan of Correction: The administration will purchase a safety sack to ensure all medication is locked.
