Comments:
An unannounced monitoring inspection was conducted on March 5, 2024 between the hours of 12:15pm-1:45pm. There were a total of 19 children with two staff members present. Children were observed during literacy time, centers, and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain the required information.
Evidence:
1. Child #2's, Child #3's, and Child #4's records did not contain documentation of the child updates and confirmation of up to date information in the child's record.

Plan of Correction: Staff will ask the parents to review the children's record and document the record for the annual review of information.
