Technical Assistance:
Technical assistance was given about batteries and staff documentation.
Comments:
An unannounced monitoring inspection was conducted on 7/18/22 from 10:11am-1:20pm. There were 7 children (19 points) supervised by 3 direct caregivers. The home, 3 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely, Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on interview and record review, a central registry check for a caregiver was not completed by the end of 30 days of employment.
Evidence: Caregiver #3 did not have a central registry check completed by the end of 30 days of employment. The Provider stated that she emailed the central registry on 3/26/22, but have not received the results. Date of employment for Caregiver #3 is 4/4/22.

Evidence: Child #4 did not have documentation of immunizations before attending the family day home. Child?s first day of attendance was 7/18/22.

Description:
Based on observation, areas of the family day home located outside was not maintained in a safe condition.
Evidence: There were two wooden logs bordering the playground equipment that were deteriorating and splintered.

Logs will be replaced by 7/20/22.
