Comments:
An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted today from 11:45 a.m. to 2:00 p.m. 70 children, ages 2 years through 5 years old, were under the care and supervision of 4 staff. Children were observed playing with toys and eating lunch prepared and brought from home. Center documents and 15 staff records were reviewed.

Description: Based on review of documentation and interview with the director, the center did not provide a written statement to the parents or guardians of the center and the general public regarding physical facilities, enrollment capacity, food services, health requirements for staff, and public liability insurance. Evidence: There was no written statement regarding physical facilities, enrollment capacity, food services, health requirements for staff, and public liability insurance accessible to the general public.

Plan of Correction: The center will post the written disclosure on the parent board in the hallway for the general public to have access to the information.

Description: Based on a review of documentation and staff interview, the center did not have a policy or procedure for ensuring that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining sand or other cushioning material under playground equipment. Evidence: There was no written policy to address obvious safety hazards and resilient surfacing under the playground equipment.

Plan of Correction: The center will create a written policy regarding obvious safety hazards and maintaining the cushioning under the playground equipment.
