Comments:
An unannounced monitoring inspection was conducted at the center between 8:00am and 10:00am. A sample of 2 staff and 5 children's records were reviewed, in addition to 1 medication. The director confirmed that was the only medication at that time. Children were observed coming into school, circle time and morning routines.
Based on a record review and interview, the center did not have annual approval from the health department. Evidence: The center's last health department inspection was dated 10/03/18. The Director stated she was aware that it had expired.
Plan of Correction:
The following corrective action was provided by the program director during the exit interview. The director will contact the health department today and schedule an inspection.

Description:
Based on a record review the center did not ensure all required background checks were complete for staff. Evidence: Staff #2 (hired 02/01/18) did not have the most current sworn statement completed.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: The director had the staff member fill out a new sworn statement on the correct form during the inspection.
