A monitoring inspection was conducted on 10/11/2022. There were 52 children present and 7 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 5 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition.
Evidence: On the playground the plastic slide was cracked at the hardware joint, a wooden board on the platform leading to the slide chute was rotted and loose and the hardware on the outside of the slide chute was rusted.
Plan of Correction:
The following corrective action was provided by the director during the exit interview: The portion of the playground equipment that has the loose board will not be used until fixed. The board will be replaced, new hardware will be installed on the slide. The director will contact the owner in regards to a plan for the cracked slide.
