Comments:
An unannounced monitoring inspection was completed on 11/05/2019 from 4:15 P.M.-6:00 P.M. At the time of the inspection 10 children were in care with one staff and one site manager. Five children records, three staff files, emergency evacuations, first aid kits, injury prevention plan, emergency supplies, emergency plan, and posted required information were reviewed. Children were observed during transition from playground to cafeteria, handwashing, small group activities that included card games, building, electronics, gross motor time in the gym, and departure. The exceptions to compliance are noted on the violation notice.

Description: Based on review of the fire inspection, the center failed to ensure that an annual fire inspection was conducted.

Evidence:
1. The last fire inspection on file was dated 09/24/2018.
2. Staff verified that there had not been a fire inspection completed since 09/24/2018.

Plan of Correction: The site coordinator will contact the school and request a copy of the fire inspection once it is completed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In the Cafeteria on a roll away cart there were two bottles of Clean on the Go HdqC2 that were accessible to children.
2. Staff verified that the hazardous substances were not in a locked location.

Plan of Correction: Staff immediately moved the cleaning solution to the janitor's closet. The site director will assign staff or herself to do a walk-thru before children arrive to the program to make sure all hazardous materials are in a locked location.
