An unannounced monitoring inspection was conducted beginning at 10:20 a.m. and ending at 12:00 p.m. with the director. There were 34 children in care ranging in age from 3 to 5 years of age under the care and supervision of 5 staff. Children were observed during morning snack, restroom break and story time. Center documents and six staff records were reviewed.

Based on review of documentation, it was determined the center did not ensure staff had documentation of repeat background checks every 5 years as required. Evidence: Staff #6, start date 2015, most recent sworn statement was dated 7/10/2013.

Description:
Based on review of documentation and interview with staff, it was determined that the center did not obtain fingerprint background check results by the required implementation date of 9/30/2018. Evidence: Staff #3, start date September 2018, fingerprint background check was dated 10/17/2018.

In the future, all background check timeframes will be followed.
