Technical Assistance:
Documentation of the arrival and departure of children in each grouping was discussed with the program director.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview and a virtual tour of the program.
A subsidy inspection was initiated on 8/19/2021 and concluded on 8/24/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined a non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Description: Based upon review of two children's records and staff interview, the facility has not ensured that they have obtained a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. The records provided for child 3 and child 4 included documentation of diagnosed food allergies. Neither record included instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergic reaction.
2. The program director confirmed that neither record included instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The facility responded with the following:
Allergic action/management plans will be obtained from physicians for all children who have diagnosed food allergies.
