Staff background checks and qualifications are kept at the main office and was not checked during the inspection.
Comments:
A monitoring inspection was conducted today between 1:00pm and 2:00pm. There were 18 children present, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed finishing lunch and getting ready for nap. A total of 5 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued. REPEAT VIOLATION Based on review of 5 children's records, the center did not maintain a complete record for each child. Evidence The record for child #1 contained the child's father information as the second emergency contact. The emergency contact must be someone other than a parent. The record for child #2 did not contain documentation of a parent's address or place of work. The record for child #3 did not contain documentation of viewing the original proof of identity.
Plan of Correction:
We will get the information from the parents. We will make sure all required information is received before a child's first day.
