Comments:
Licensing Inspector arrived at 12:45 and departed at 2:15 PM to complete a religiously exempt code compliance inspection. At the time of the inspection, there were 47 children (ages infant through school age) in care with six staff. Children were observed napping. All requirements related to religiously exempt facilities were reviewed.
Based on a review of 13 staff records, it was determined that the facility did not ensure that all persons had a completed search of the central registry on file within 30 days of employment.
Evidence: None of the staff identified as working at the facility had a completed search of the central registry on file.
Plan of Correction:
All searches were submitted, but then returned as the check was made out incorrectly. All have been resubmitted and will be placed in the staff records.

Description:
Based on a review of 13 staff records, it was determined that the facility did not ensure that all staff who have lived outside of Virginia within the past five years have a completed search of the central registry for states where they have lived on file.
Evidence: 1. The sworn statement of affirmation for staff #6 indicated that she has lived in North Carolina within the past five years. There was no search of the central registry for North Carolina on file. 2. The sworn statement of affirmation for staff #13 indicated that she has lived in Ohio within the past five years. There was no search of the central registry for Ohio on file.
Plan of Correction:
Out of state checks will be completed and placed in staff records.
