Comments:
A religious exemption renewal inspection was conducted on 4/29/2022. There were 59 children in care, with a total of 14 staff assigned to their direct supervision. The children were observed completing puzzles, playing outside, engaging in circle time discussions, enjoying classroom creative play activities, and eating lunch from home. The Inspector reviewed compliance in the areas of background checks, the physical plant, administrative records, and children's health records. A total of 6 children's records and 5 staff records were reviewed.

Based on a review of staff background checks, it was determined that two staff involved with the preschool operations have not provided a signed sworn statement. Evidence: Classroom staff C and administrative staff B have not signed and dated a Sworn Disclosure Statment.

Both will complete their Sworn Statement next week when they return.

Based on a review of staff background checks, it was determined that the preschool has not obtained the results of a search of the central registry for one staff person involved in the preschool operations. Evidence: A search of the Virginia Central Registry has not been completed for administrative staff B.
