Comments:
An unannounced renewal inspection was conducted on September 27 2019 between 2:00pm and 3:30pm. There were 20 children in the direct care of 2 staff members during the inspection. Children were observed playing on the playground equipment. Staff records, Board officer records, evacuation drills, training hours, physical plant, and the fire inspection were observed and inspected. Based on record review, 2 of 5 staff records reviewed did not contain documentation that at least every two years from the date of the first initial screening staff submit the results of a follow-up tuberculosis screening.
Evidence: Staff #3 record has a documented tuberculosis screening dated 09/16/2015. Staff #5 record has a documented tuberculosis screening dated 10/8/2015

Based on record review, 4 of 4 agent records reviewed did not have documentation that a fingerprint screening had been completed.
Evidence: Agents listed on the application had not completed the fingerprint requirement that was to be completed no later than September 2018. Each agent did have a current criminal record check on file.
