This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.
A code compliance inspection was initiated on 04/05/2022 and concluded on 04/05/2022. The principal was contacted via video call to initiate the inspection. There were 9 children present and 1 staff. The Inspector reviewed 2 staff records and 2 children's records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable code sections and a violation was documented on the violation notice issued to the facility.
Based on record review, the center did not ensure to maintain central registry results every 5 years for two of two staff records. Evidence: 1. The record of staff #1 (start date: 8/2016) had documentation of central registry results dated 8/15/2016. 2. The record of staff #12(start date: 7/2016) had documentation of central registry results dated 8/9/2016.
Plan of Correction:
We will get those central registries updated within the next 30 days.
