Comments:
An unannounced on-site monitoring inspection was conducted on 09/20/2023. There were 44 children, ages 5 years-12 years, and 4 staff members present during the inspection. The inspector observed the following: arrival into the program, snack and, large motor play outside. A total of 5 children?s records and 3 staff records, were reviewed. The inspector arrived at 3:30 PM and departed at 4:45 PM.

Based on review of six staff records, the center failed to obtain the finding from a search of the central registry within 30 days of hire.
Evidence:
1. Staff # 3 had a hire date of 09/18/2022. A return search of the central registry was dated 11/05/2022.
