Comments:
An unannounced monitoring inspection was conducted on September 26, 2023 with the center director. There were 39 children present, ranging in ages from 5 months to 4 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to ensure that all employees shall undergo background checks every five years. Evidence: 1. The staff record for Staff #3 contained a national criminal background check dated 7/9/2018. Staff #3should have undergone a criminal background check on or before 7/9/2018. 2. The director verified that a criminal background check had not been completed for staff #3 since 2018.
Plan of Correction:
Will get fingerprints done immediately.
