Comments:
An unannounced monitoring inspection was conducted on 3/22/2024 between the hours of 9:00am-11:00am. There were 37 children directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and injury reports were inspected. Children were observed playing outside and having Easter-themed snacks. Handwashing was also observed. Areas of non-compliance are identified in the Violation Notice. Based on record review, 1 of 5 staff reviewed did not submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment.
Evidence: 1. The tuberculosis screening in the record for Staff #1 (employment date 3/15/2024) was dated 1/24/2024.
Plan of Correction:
The staff member will obtain a new tuberculosis screening. Tuberculosis screenings for new staff will be obtained within required time frames.

Evidence: 1. An injury report for Child #4 dated 09/08/2023 did not contain the time when parents were notified, and the documentation on how parent's were notified.

The center will make sure all information is documented on the injury report form.
