This inspection was conducted by licensing staff using both an alternate remote protocol, including telephone contacts, documents reviewed, review of video footage and interviews .
An self report investigation was initiated on August 26, 2021 and concluded on October 15, 2021. The investigation was in the area of staffing and supervision and special care provisions and emergencies. The director was contacted by telephone, interviews were conducted, video footage was reviewed.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
