Comments:
An unannounced monitoring inspection was conducted today. There were 51 children in care, with a total of 5 staff members assigned to their direct supervision. Children's records, children's medications, emergency preparedness items, the physical plant, children's nutrition, and classroom programming/interactions were inspected today.
The children were observed returning from open gym play activities, playing card games, and using electronics from home before eating lunch. All children brought lunch from home today.

Based on a review of children's health records, it was determined that one record does not include documentation of the child's physical exam reported on the current form required by the Virginia Department of Health. Evidence: Child record B does not include documentation of a full physical exam on the VA Department of Health form. This record only includes a brief health summary completed on a form obtained from a different public school system.
Plan of Correction:
The parent will be asked for documentation of the full physical completed on the correct form.

Description:
Based on a review of children's records, it was determined that three records do not include all required information. Evidence: Child records A and D do not include the contact information for two emergency contacts other than the child's parents. Child record C does not include the father's work phone number.
