Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on and concluded on 2/10/2021. The site supervisor was contacted by telephone to initiate the inspection. There were 25 children present and 12 staff. The inspector emailed the site supervisor a list of items required to complete the inspection. The Inspector reviewed 4 children?s records to ensure documentation was complete.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on a review of children's records, it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child A entered care on 9/8/2020. There is no record of immunizations in this child's file.

We will ask the school nurse or the parent for the shot record.

Description:
Based on a review of children's medications, it was determined one authorization form was not completed with all required information. Evidence: Child D has an authorization for one long term emergency prescription medication. This authorization does not include a documented start and stop date for which the authorization is effective.
Plan of Correction:
The parent will be asked to have the doctor add the information.
