Discussed with Center Supervisor: *Use of the Art Room - safety concerns *Annual Health and Safety Update training - if taken in 2020 or 2021 does not need repeated until January 2023 per the DOE website.
Comments:
An in-person monitoring inspection was conducted today. There were 3 groups of children present (26 children) with 3 staff. Violations were discussed with the Center Supervisor (CS) and a plan of correction was provided to the LI. Medications, emergency evacuation drills, emergency supplies and first aid supplies are all in compliance. Children were observed participating in a variety of activities after school. Based on a review of records, one child record did not contain documentation of a completed physical within 30 days of enrollment. Evidence: 1 of 3 records reviewed did not contain a physical. (Date of Enrollment: 12/10/21) (Child 1)

I will pick up the physical from the front office tomorrow.

Description:

Description:
Based on direct observation hazardous substances were not kept locked. Evidence: Room 1 had hand sanitizer in an unlocked cabinet. Room 2 had isopropyl alcohol, hydrogen peroxide, saline solution and shaving cream in an unlocked cabinet.
Plan of Correction:
All products were moved to a locked location during the inspection.
