Comments:
An in-person monitoring inspection was conducted on April 21, 2023 from 2:30pm until 3:00pm. There were 29 children with 3 staff supervising. One group of children were observed playing in the gym with a teacher lead activity. Another group of children were playing small group table games and helping the teacher in their classrooms. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not ensure all children's record contained all required information .
Evidence: Child 1, 2, and 3 are missing parent work information.
