Comments:
An unannounced renewal inspection was conducted October 18 2019 between 8:00am and 9:30am. There were 26 children observed in the care of 5 staff members. Children were observed reading, playing outdoor sports, creating art, and playing with various games in the center. The center is clean and organized. Based on record review, 2 of 5 children's records reviewed did not have documentation that staff requested at least annually parent confirmation that the required information in the child's record is up to date.
Evidence: Child #1 and Child # 5 record did not have documentation that a request had been made to parents.
Plan of Correction:
Staff will have the parents update the information.
