Comments:
An unannounced monitoring inspection was conducted before school today. There were 21 children with 2 staff by the end of the visit. 4 child records were reviewed. See violation notice for non-compliances cited today. 2 staff records were reviewed for specific items only, as the full record is reviewed at the main office at another time. Emergency evacuation drills, shelter-in-place drills, emergency supplies, first aid supplies, posted documents and age appropriate activities were all in compliance. Violations were reviewed with staff present. Based on a review of records, one child record did not contain documentation of a completed physical within 30 days of first attendance. Evidence: 1 of 2 records reviewed did not contain a completed physical. First date of attendance: 1/16/23 (Child 2)
Plan of Correction:
I will check with the school office to see if they have a copy of the physical. I will ask the parent to provide one if the office doesn't have one.
