Comments:
Conducted an unannounced inspection at 11:40am. Observed 23 children + 3 direct-care staff. Ratios + supervision were in compliance. Children were observed playing water games on the outdoor playground. Staff were actively engaged with the children + activities. The program areas were well supplied with games + equipment for the children. An area of non-compliance with standards + laws reviewed was found. Based on records reviewed, it was determined that 1 of 2 children's records did not include a physical exam. Evidence: The record for Child #8 did not include a physical exam.
Plan of Correction:
A copy of the physical exam will be put in the child's record.
