A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. An area of non-compliance of the supplemental SHSI requirements was found. Evidence: Staff #7-The training certificate on-file was dated as completed 4/27/22. Plan of Correction: If the course was not completed, the staff person will complete it promptly. If the course was completed, documentation of training will be placed in the staff record.
