Comments:
An unannounced Monitoring Inspection took place during a Teacher Workday on 10/24/2022 between the hours of approximately 11:10 a.m. and 11:30 a.m. There were 2 classrooms observed with a total of 24 children with 3 staff within the supervision guidelines. The children were observed preparing to go outside and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain all information for children?s records.
Evidence: 1. Child #1?s record did not contain the name, address and phone number of two designated people to call in an emergency. 2. Child #2?s record did not contain the address for a second emergency contact.
